Mission of Steps to a Healthier Clark County Health Systems Teams - To work with Health System organizations to improve standardization of prevention messages and practices administered to community leaders.

Health care systems are acutely aware that chronic diseases continue to drive up health care costs and increase the number of individuals without health insurance.

Because the providers in our health system see, on a daily basis, the impact of these chronic diseases, proactive efforts to build awareness is one role for health care providers in Clark County

One way this is happening is through the Washington State Diabetes Collaborative. Two clinics in Clark County, Family Medicine of Southwest Washington and the Family Wellness Center, have been case managing their patients with diabetes with the assistance of Steps funding. The goal of this team, which includes the patient, the physician and clinical staff, is to prevent and reduce complications of diabetes. This team effort by each clinic requires the tracking of patient data and diabetic indicators, which include:

Hemoglobin A1c
Blood Pressure
LDL
Self Management goal
Tobacco Cessation Counseling

After 12 months, the majority of the patient indicators show improvement and an important lesson learned by the health care teams is "What gets measured gets done".
